1. Enter your details in the form opposite
You will be shown a link to download the registration form. You will also be emailed a link to download the form so you can download it later at your convenience.
2. Once downloaded, print and complete the registration form, attach all relevant documentation, such as proof of qualification and insurance, and enclose payment by cheque and send to:

Your application will be processed and verified within 10 working days and once you receive an email confirming acceptance of membership you will then have access to the additional pages in the Members Area.
4. If you have been accepted as a Full or Graduate member and you wish to appear on the Find a Practitioner page, you are now able to submit your profile and practice details and submit for publication under the Profile Update page.
Your profile and practice details will be published on the website within 7 working days of submitting your form.
